Sajda Sahu for recitation of Qiraat

Please advise if Sajda Sahu should be performed for each of the following 4 cases, all relating to the recitation of Qiraat after Surat Fatiha. If one makes (A) mistake during the first 3 Ayats? (B) forgets during the first 3 Ayats? (C) mistake after the first 3 Ayats? (D) forgets after the first 3 Ayats?
Sajda Sahw is not wajib due to forgetting: no matter if it is in the first three (3) ayats or after the first three (3).
If the person reciting the Quran has made a mistake, it needs to be ascertained as to what type of mistake has been made. If it is such a mistake that changes the meaning of the ayats then no matter it is in the first three (3) ayats or afterwards the salaat has been negated. If the mistake does not change the meaning then it does not negate the salaat, no matter if the mistake is in the first three (3) ayats or not.
