Concentrating in Prayer (Salat)

I find it difficult to concentrate in my prayer (salat). At times I forget whether I have done the simplest of things, such as one prostration or two. Sometimes, I have repeated the prayers in which this has happened, but now, it occurs so often, even in the repeated prayers, that I have stopped repeating the prayers. Is what I have done correct? Is this due to waswasa?
