Am I entitled to a divorce

I married a muslim man and converted to Islam several years ago.  We have a young son together as well as my child from a previous marriage.  During the previous 3 years the marriage has become more difficult with my husband becoming physically and more mentally abusive towards me and my teenage child.  

He continues to call both her and I bad names and has physcially punched me, thrown me to the ground and kicked me in the stomach after an argument.  He would spend many hours on his phone and withheld relations from me as his wife for reasons of being tired and saying he did not want to be intimate with me.  He would stay for hours on his phone during the night and would put a password on his phone.  He started going to fix things for a muslim lady and many times was alone in the home with the woman and her daughter who began to text him on a regular basis.  

He started to visit frequently and would stay until late at night and hurry home and shower before praying.  I did not know and would accuse him of cheating after finding residue from intercourse in his underwear and he continued to deny.  I let this go and continued to endure the marriage.  There are some more things which I cannot say on an online forum but would like to speak privately with someone on my rights as a wife and if I am entitled to a divorce.  The other thing is very very bad and I cannot write.
