I am doing business related to doctors. I give commission for the patients refer by the doctors, if the referred patients purchase the product from our shops. We were giving out some percentage from our profits to the doctors as commission. For this we will not sell the products to the doctor referred patients at higher rates. All rates are fixed and we sell all our products at uniform prices to all our customers. Paying cash to the doctors as commission in this regard is halal or haram.

The amount (commission) given to the doctors apparently is not the compensation of anything as the doctors receive their compensation in the form of fee hence this amount seems bribe. Hadith sternly curses those who take bribe as well as those who give it.
لعن رسول الله صلى الله عليه وسلم الراشي والمرتشي (ابوداؤد 3580)
Hence one should avoid taking such kind of commission as long as one can.
