Is it permissible for a woman to get breast reduction surgery who suffers from back pains due to having large breasts?

Answer:
Altering the face and body through surgery is not permitted if done for cosmetic purposes, however, in cases of genuine medical need, there would be scope for permissibility.
If your condition of back pain has been diagnosed by an (ideally Muslim) medical professional to be attributed to large breasts and they have recommended this procedure, and if there is no other alternative that will be as effective in reducing your symptoms, and the benefits of the surgery outweigh any other potential risks and side effects, it will be permitted for you to do.
