Covering the Face in Prostration
Even though having clothing on one’s forehead and/or nose is somewhat disliked – due to it being a barrier between the skin and the ground – it does not affect the validity of the prayer. [Shurunbulali, Maraqi al-Falah]
You should try to keep some portion of your forehead uncovered, even if little.
It is also disliked for one to cover their nose, mouth, or both in the prayer. [Ibid.]
As for the hands, there is no problem with the hands being covered.
Errors in the Remembrances of the Prayers
Regarding errors in pronunciation many scholars, have deemed such mistakes, whether in the recitation of the Qur’an or other, to be pardoned. This is due to a lack of knowledge of the rules becoming widespread. [Ibn ‘Abidin, Radd al-Muhtar citing al-Hinduwani, al-Halwani and others]
