If a muslim man lives with his girlfriend as man and wife but does not marry her, how is this regarded in Islam.

Since Nikah has not been conducted, they are living in sin. This is Zina
(fornication) which has been vehemently abhorred by the Qur’aan.

A simple Nikah that wouldn’t even take a few minutes will prevent them from
engaging in sin.
