Should we say “bismillah” before Surah Fatiha in every rakaat?

Are you supposed to say “Bismillah” before Surah Fatiha in every rakaat? what dua should be read between two sajdas?

Yes, in every rakat a person should recite bismillah prior to reciting the sura fatiha and a person should recite allahum-ma-ghfirli in between the two prostrations while sitting.
