Divorce has not happened but husband did not mention the name of spouse on official papers and leave space blank or nil. But in real he has no purpose to separate from his wife. Wife or her guardians did not know about such papers. Will this be considered as talaq?

For talaq to take place, it is necessary to write or utter the words of talaq. Talaq did not take place by leaving the space for wife name blank in the official papers.
