Please advise if Sajda Sahu should be performed for the following 4 cases, If one makes a (1) mistake (2) forgets (A) during the first 3 Ayats? (B) after the first 3 Ayats?

Answer:

There is no Sajda-e-Sahw when one makes a mistake in Qiraat or forgets the
Qiraat.
