Reciting without topi (cap)

(a) Is it allowed to read the Quran without topi (cap) and also lien with the wall? (b) Is it allowed to keep reading the Quran while the azan is being called ? (c) If by mistake the Quran falls down then what should be done (sadqa or not )?
1.It should not be made into a habit, but if someone is walking, or reciting the Quran, casually, from memory, then there’s nothing wrong in not having the head covered.
2.The Quran can be recited while the azan is being called, but it’s better to, first, stop reciting, answer the azan, and then continue reciting.
3.If the Quran falls by mistake, we should ask forgiveness from Allah (do Istighfar). It’s also been recommended by the scholars that we should give some sadaqah (voluntary almsgiving for the sake of Allah), so that we may feel some regret for our action due to this monetary loss, causing us to have a strong intention not to have it happen again in the future. Because most of the time these types of mistakes occur due to carelessness, by punishing ourselves (monetarily or otherwise) we would be motivating ourselves to be more careful next time.
