Does the injecting of insulin break the fast?
Answer:

In the Name of Allah, the Most Gracious, the Most Merciful.

As-salāmu ‘alaykum wa-rahmatullāhi wa-barakātuh.

If one injects insulin directly into the stomach, then the fast will break.  However, if it is injected in areas other than the stomach like the upper arms, upper buttocks or the outside of the thigh, the fast will not be break.[1]

[1]  والمفطر إنما هو الداخل من المنافذ (رد المحتار ج 2 ص 395 أيج أيم سعيد)
والذي ذكره المحققون أن معنى المفطر وصول ما فيه صلاح البدن إلى الجوف أعم من كونه غذاء أو دواء (رد المحتار ج 2 ص 410 أيج أيم سعيد)
وما وصل إلى الجوف أو إلى الدماغ من المخارق الأصلية كالأنف والأذن والدبر بأن استعط أو احتقن أو أقطر في أذنه فوصل إلى الجوف أو إلى الدماغ فسد صومه (بدائع الصنائع)
إن داوى جائفة ـ أي الجراحة التي في البطن ـ أو آمة ـ أي الجراحة في الرأس ـ فوصل الدواء حقيقة إلى جوفه ودماغه، فعليه القضاء إن كان الدواء رطباً أو كان يابساً وعلم أنه وصل إلى جوفه أو إلى الدماغ، أما إذا كان الدواء يابساً ولم يعلم بوصوله فلا قضاء عليه( الجامع في أحكام الصيام والاعتكاف والحج والعمرة ص 57 ج طبعة دار الجنان)
