When a masbuq joins Salah in four Rakah and stands up after the Imam says ‘salaam’, does he say the takbeer before recitation of Surah Fatiha (of 1st Rakah)?

I recently learnt that i have been praying fard wrongly. after the first two rakah I did not know that you do not read a surah after surah fatiha. does this invalidate all my fard salaat.

While performing witr salat, I read a specific surah in the first and second rakah. Does the salaat have to be repeated? Can the same surah be recited in different rakah for any salaat?

I realised the mistake after I had made takbir for dua-e-qunoot. Sajda Sahw was performed thereafter, as the mistake was realised. Does the salaat have to be repeated?
