Fasting while using a hemorrhoid cream

While fasting are you generally allowed to use external medication like a cream? More specifically, can one use a cream for hemorrhoids?

Answer:
In the Name of Allah, Most Gracious, Most Merciful

Using creams on the outside of one’s skin does not invalidate the fast, even if the skin absorbs it, because what invalidates the fast is a substance reaching one’s inside (body cavity) through an open orifice such as the mouth, the nose and the anus.

Likewise, it has been said that one who uses a cream to treat hemorrhoids on the outside of the skin, does not invalidate the fast. But if he uses it on the inside of the anus, it does invalidate the fast in our school, because the substance has reached the inside of an open orifice, causing it to be invalidated.

However, if the one with hemorrhoids needs it for treatment, and he is not able to delay it until nighttime, because delaying would delay his recovery, or increase his pain, for example, then it is permissible for him to use it inside the anus, even during the day of Ramadan.

This would invalidate his fast, but he is excused because of the illness so he is not a sinner. He must remember to make up the days he missed because of it.

السؤال: هل الإستطباب بالكريمات العلاجية بشكل عام ، وبالكريمات العلاجية للبواسير بشكل خاص ، يُبطِلُ الصيام ؟

الجواب : استعمال الكريمات على ظاهر الجلد لا يبطل الصوم ولو تشربها الجلد ؛ لأن المبطلَ وصولُ عين إلى الباطن من منفذ مفتوح كالفم والأنف والدبر ، وكذلك يُقال فيمن يستعمل الكريمات لعلاج البواسير إن كان استعمالها على ظاهر الجلد فلا يبطل الصوم ، أما إن كان استعمالها بإدخالها باطن الدبر فهو مبطلٌ للصوم في مذهبنا ؛ لما عرفتَ من أن ذلك عينٌ وصلت الباطن من منفذ مفتوح فتبطل ، لكن إن احتاج صاحبُ البواسير إلى العلاج بذلك ولم يمكنه تأخيره إلى الليل لأن تأخيره يؤخر الشفاء أو يزيد الآلام مثلاً فيجوز له استعماله في باطن الدبر ولو في نهار رمضان ويُفطر به وهو معذورٌ بفطره لأجل المرض فلا يأثم ، لكن يقضي ما أفطره من الأيام .
