My current employer offers 3 different health care benefit packages. Core is the most basic with $0 health premium paid by employee (my self), Enhance is medium plan where employee pay around $34 / month health premium. And Elite is the top package with $81 / month price tag. LTD (Long Term Disability) premium is must regardless of which plan I choose. The deductibles for all the 3 plans are taken from my salary prior of I receive it. Please find a comparison table below:
Kindly suggest which plan & benefit is complying with Deen. I would like to mention I’m based in Canada.

As per the options described in your question, the best option is where you do not have to pay anything. There is gunjaish(provision) for (permissibility of) the other options mentioned as well. However, the options where you have to pay the share, if you get medical treatment then it will be impermissible to accept cash reimbursement for such treatments. Instead, in return of the share you have paid, the correct way is for the employer to manage the payment for your treatment.
