How Should the Hands be Raised?

When one is beginning the prayer, how should the hands be raised to the earlobes? Should the thumbs touch the earlobes? Should the palms be facing qibla?

Is this the same for both men and women?

Women must raise their hands in line with their shoulders.

NB: Recently an excellent book was published by Madrasah Arabia Islamia, Azaadville, South Africa. titled: “The Salah of Women.” It is beneficial to both the beginner and the scholar alike. It discussed the manner of salah for women as outlined by the Hanafi jurists in the light of the hadith and the athar of the Sahabah and Tabi`in citing the Arabic quotations with full references.  It also refutes the view of certain Salafis and Ghayr Muqallidin who claim that there is no difference in the salah of men and women.
