Answer
Bismillah

Al-jawab billahi at-taufeeq (the answer with Allah’s guidance)

If a Taweez contains words of Kufr and Shirk or one regards the Taweez to be the ultimate Curer, then that is Shirk.

If a Taweez that has verses of the Qur’aan and does not regard the Taweez to be the ultimate curer, then that is permissible and it is not Shirk.

In the Quran there is (spiritual, physical) cure for people. Allah Ta’ala says in the Noble Quran,

“We sent down of the Quran that which is a healing and a mercy to believers.” (17:82)

And Only Allah Ta’ala Knows Best.
