Answer
Bismillah

Al-jawab billahi at-taufeeq (the answer with Allah’s guidance)

If the need of taking the tooth out is necessary then it will be permissible. But, if it is not necessary then it will be Makruh (disliked). (Ahsanul Fataawa Vol. 4 Pg. 436)

The fast would not break by taking the tooth out but it would nullify the fast if one swallows the blood from the gums, if the colour of blood is more than the saliva or equivalent (this will also nullify the Wudhu) or it’s taste being felt.
(Aapke Masai’l Aur Unka Hal Vol. 3 Pg. 290, Ahsanul Fataawa Vol. 4 Pg. 436)

It would therefore, be advisable to delay the appointment until after Ramadhan.

And Only Allah Ta’ala Knows Best.
