Reciting a Surah in the third and fourth rakaat of fardh Salaah

Q: My question is if someone unintentionally read surah in third or fourth rakaat of farz salaah, does he has to do sajda sahw? Similarly if someone forget to read surah in third or fourth rakaat of sunnat salaah, does he has to do sajda sahw?
