Can you confirm the source of this du’a?
اللهم إني أعوذ بك من عذاب جهنم وعذاب القبر وأعوذ بك من فتنة المسيح الدجال وأعوذ بك من فتنة المحيا والممات

Imam Tirmidhi (rahimahullah) has recorded this du’a on the authority of Sayyiduna ‘Abdullah ibn ‘Abbas (radiyallahu ‘anhuma).
(Sunan Tirmidhi, Hadith: 3494)
Imam Muslim (rahimahullah) has recorded the du’a with slight variation in the wording:
Sayyiduna ‘Abdullah ibn ‘Abbas (radiyallahu ‘anhuma) reports that Nabi (sallallahu ‘alayhi wa sallam) would teach them [the Sahabah radiyallahu ‘anhum] the following du’a just as he would teach them a Surah from the Quran. He would instruct them to say:
اللهُمَّ إِنَّا نَعُوذُ بِكَ مِنْ عَذَابِ جَهَنَّمَ، وَأَعُوذُ بِكَ مِنْ عَذَابِ الْقَبْرِ، وَأَعُوذُ بِكَ مِنْ فِتْنَةِ الْمَسِيحِ الدَّجَّالِ، وَأَعُوذُ بِكَ مِنْ فِتْنَةِ الْمَحْيَا وَالْمَمَاتِ
Allahumma inna na’udhu bika min ‘adhabi Jahannam wa a’udhu bika min ‘adhabil qabri wa a’udhu bika min fitnatil masihid dajjali wa a’udhu bika min fitnatil mahya wal mamati
(Sahih Muslim, Hadith: 590)
Translation
O Allah! We seek Your refuge from the punishment of Jahannam, and I seek Your protection from the punishment of the grave, I seek Your protection from the trials of Dajjal and I seek Your protection from the trials of life and death
