What does the Hadith say about raising hands in the third raka’at of Witr salat and reciting the Du’a-e-Qunut?

Is there any Hadith where it is proven to raise hands in the third raka’at of Witr salat and recite the Du’a-e-Qunut? Please give the Hadith with its translation and the name of the Hadith book with the name of the chapter [where this Hadith is found] and also the number of the Hadith.

عن الأسود عن عبد اللہ انہ کان یقرأ فی آخر رکعۃ من الوتر قل ھو اللہ احد ثم یرفع یدیہ و یقنت قبل الرکعۃ (اخرجہ البخاری فی جزء رفع الیدین)

Aswad narrated from Abdullah that the Prophet (Sallallahu Alaihi Wasallam) used to recite Surah Ikhlas in last rakat of witr, then he used to raise his hands and recite qunoot before ruku. (Bukhari, Juzu Raf-il-Yadain)

واثر ابراھیم النخعی بسند صحیح عند الطحاوی: ترفع الأیدی فی سبع مواطن؛ فی افتتاح الصلاۃ و فی التکبیر للقنوت فی الوتر الخ (اعلاء السنن؛ 6/101، ط باکستان)
