Answer:
In the Name of Allah, Most Gracious, Most Merciful

Assalamu alaikum wa rahmatullah

Forgetfulness prostrations are necessary (wajib) to perform for leaving any of the necessary (wajib) actions of the prayer forgetfully. It is not performed for anything else–whether deliberate omission of necessary actions, or the omission of sunna actions, or the performance of something disliked.

Forgetfulness prostrations are optimally performed after one salam at the end of the prayer. They consist of two prostrations–performed like the prostrations of prayer. After them, one repeats the final sitting in full: reciting the tashahhud, sending blessings upon the Prophet (peace and blessings be upon him), and making supplications before ending the prayer with two salams, as usual.
